Why did I receive this check or eGift Card?
You received a check or eGift Card because you submitted a valid and timely claim in the Online DVD Rental Antitrust Settlement, in which you requested either a cash payment or gift card.  After payment of attorneys’ fees and costs, payments to Class Representatives, and administrative costs, the remaining Settlement Fund was divided equally amongst Class Members who filed valid claims.


How do I get my check reissued?
If you recently received a check and wish to have the check reissued please review the options below carefully to understand what is needed in order for the check to be reissued.  Requests to reissue a check will be accepted by the Claims Administrator until February 3, 2016.

Option 1 - If you are requesting that the check be reissued due to misplacing it, or accidently destroying the check, please mail a signed letter with your name as it appeared on the check and the reason you need your check reissued, to the Claims Administrator at the address below.

Option 2
- If the person listed on the check is deceased, the following items must be sent  together to the Claims Administrator in order to have the check reissued: 1) a copy of the death certificate; 2) documentation showing your authority to act on behalf of the deceased (for example, a copy of the letters of administration or other court documents); 3) a signed letter with your name, the check recipient’s name, who the new check should be issued to and why; and 4) the voided check if you have the check, at hand.  Please mail all of the above requested documents to the Claims Administrator at the address below.

Option 3 - If you are requesting that the check be reissued because of a name change due to marriage or divorce, or any other reason than previously described, please mail a letter to the Claims Administrator noting your maiden name or the name that is currently on the check, your current name or the name that a new check should be reissued to, a detailed and signed explanation why the check needs to be reissued, and a copy of the legal documents showing the name change.  This may include a marriage certificate or divorce decree, if applicable.  Please mail all of the above requested documents to the Claims Administrator at the address below.

Option 4
- If you are requesting that the check be reissued because you have been given a Power of Attorney by the check recipient, we are unable to change the name listed on the check.  If the Power of Attorney document is valid, you should be able to cash the check with the recipient’s name listed on the check by presenting proper documentation at the bank.


I lost the email containing the link for my eGift Card.  How can I get a new email or link?
If you submitted a valid claim and elected to receive a walmart.com eGift Card, you should have received an email message that looks like the one below.



If you require your eGift Card to be resent, please mail a signed letter to the Claims Administrator requesting to receive your eGift Card again.  Requests to reissue an eGift Card will be accepted by the Claims Administrator until February 3, 2016.  The letter should include your name, mailing address, email address and the reason for having the eGift Card resent, and be mailed to:
